Charges: The total amount a healthcare provider bills for a particular medical service or procedure.

Allowed Amount: The maximum amount that a health insurance plan is willing to pay for a specific covered healthcare service. It may be less than the provider's charges.

The amount you pay for covered healthcare services before your insurance plan starts to pay. With a $2,000 deductible, for example, you pay the first $2,000 of covered services yourself. After you pay your deductible, you usually pay only a copayment or coinsurance for covered services.

A copayment, or copay, is a fixed amount for a covered service, paid by a patient to the provider of the service before receiving the service.

The maximum amount of money an individual must pay for covered healthcare services within a specific period (usually a year) before the insurance plan covers 100% of the remaining costs. This includes deductibles, copayments, and coinsurance.
