PIAA Physicals

As part of our dedication to keeping area athletes safe, Orthopedic Associates of Lancaster proudly offers PIAA physicals for student-athletes in grades 7 – 12.

All physicals are performed by board-certified providers from Orthopedic Associates of Lancaster at our offices or the respective school if not otherwise noted. If an athlete cannot attend their school’s date, please schedule a separate appointment with one of our providers.

PIAA physical forms:

• Bring your completed PIAA physical packet to your appointment
• Bring a completed Orthopedic Associates of Lancaster Consent to Treat Form
